Who Should Attend:
Physicians interested in health care homes and how they are implemented in Minnesota.

Objectives:
Following attendance at this presentation, participants should be able to

• Describe the medical home concept, national efforts, and evidence from demonstration projects
• Summarize Minnesota’s health care reform/health care home legislation and how Minnesota is advancing the legislation
• Describe how the state will determine readiness and certify health care home
• State objections to and support for various criteria a practice must meet to be certified as a health care home

CME Credit:
The Minnesota Medical Association is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The MMA designates this educational activity for a maximum of 3.5 AMA PRA Category 1 Credits^TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.
